We accept most traditional insurance plans. You can contact Pleasant Avenue Dentistry to verify acceptance of your plan. We do not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you. We accept checks, cash or credit cards. We also offer a flexible payment plan and invite you to contact us to learn more about our in-house payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.

Payment Policy
Payment is due at the time of treatment. We accept cash, checks and any major credit cards. We also have a payment plan called CareCredit® that allows you to start treatment today and spread payments over time, with the possibility of interest-free time.

Cancellation Policy
We understand that sometimes appointments may need to be rescheduled. We request that you give our office at least a 24-hour notice that you will be unable to keep your scheduled appointment. At that time, we will help you to find a better time and date to accommodate you and then reschedule your appointment. If for any reason you cannot give a 24-hour notice, you will be charged a fee of $50.00 or more, depending upon the particular treatment that was scheduled for that appointment. We reserve specific amounts of time in our schedule for our patients, ensuring that they will receive the proper treatment and care during that time. Our patients are very valuable to us, as is our time with them. We ask that you please be considerate.

Discount Policy
Our dental office provides discounts for pre-paying in full with cash or checks for scheduled dental appointments for amounts over $500.00. Discounts as followed:

• 5% discount for pre-paying with cash or check prior to day of service

CareCredit®
If you need help paying for dental treatments, we may have an answer for you. Our office has partnered with CareCredit. This is a third-party company that may be able to finance the entire cost of your treatment with low or even no down payments or interest rates.

 

Private Dental Plan
Even if you do not have insurance, you can still get the care you need from our dentists, Dr. Ben Sitz, Dr. Elise Soulliere or Dr. Carolyn Disse. When you call our office at 218-237-7200 for your dental appointment in Park Rapids, Minnesota, be sure to ask about our Private Dental Pan. You can enjoy dental coverage without insurance.
